It's so hard to know where to begin with an experience that was this special on so many levels. If you're called by the medicine, look no further. Arkana is many things; a sanctuary of healing, a five star resort (with beautiful grounds, a great pool, and world class food) a magical ewok village, and a miracle. Every detail of the experience is completely dialed in and optimized so you can focus entirely on your healing. The level of care and the intelligence in the approaches are top-notch and incredible. The staff is there for you all day every day and do everything to ensure your experience is as focused and healing as possible. I cannot recommend highly enough and will always be grateful for my experience.

To Finalize the Booking:
1. Upon completion of this form your space will be reserved.
2. Please complete the medical intake form in the next screen.
3. An email with a link to complete this form will also be sent to all other guests in your reservation. If they cannot find it, please have them look for it in the Promotions or Update folders.
4. All medical intake forms are expected to be completed within 72 hrs of registration. Reservations will be canceled and spots released if we don't get your form on a timely manner, your cooperation is greatly appreciated.

Important: Please DO NOT book any flights until you have received confirmation that we have received your medical form. If there is any medical reason that disqualifies you for the retreat, you are entitled to a full refund of your deposit.

Cancellation Policy *	I understand that if I cancel my reservation, a personal credit will be issued for any future retreat at any Arkana center in the next 2 years. The amount of credit will depend on the timing of the cancellation advice as per the terms and conditions.

Contraindications *	I attest that none of the applicants in this form have been diagnosed with Bipolar condition (I or II), Schizophrenia, Schizoaffective Disorder, Borderline Personality, Psychosis or Epilepsy.
Plant Medicine is not included in the price *	Participants acknowledge and agree that retreat fees presented and payable pertain solely to the cost of accommodations, meals, workshops, medical onboarding, and local transportation; they DO NOT include any cost for plant medicine, psychedelics or similar products. Cost for Shamanic Ceremonies are to be paid in cash to a local third party, no credit card may be used for this.
Payments do not include the cost of plant medicines. *	I understand that payments made through Retreat Guru do not include the cost of: plant medicine, psychedelics or similar products.
